So I've picked up Pesti (again), and I've been sticking to the item build suggested in the premium guide for him, and I tend to agree with the items the guy has chosen over what some other people tend to do which is lifesteal.
That being said, I tend to go logger's -> steamboots -> insanitarius -> Tanking item
The tanking item can be 1 of 3 choices:
- Shrunken Head: My first choice for most games as people tend to focus pesti if he gets disabled
- Shaman's Headdress: For a magic heavy team that doesn't necessarily have much in the way of disables
- Daemonic Breastplate: Almost never get this item first, if ever before the game ends. Getting it first means they have a heavy physical damage with few, if any immobilization skills
So my question is: should I swap the order in which I get my items? I was thinking instead of going insanitarius -> tanking item, I could go tanking item -> insanitarius. I also feel this is kind of game dependent, but if I'm getting hammered pretty heavily, insanitarius really helps my ability to farm and gives me a minor (but not insignificant) boost to my survival, whereas shrunken head, for instance, is a huge boost to survival with a moderate increase to my ability to farm.

My Item for Pesti is:
Powersupply (damn it saves whole time your ass :P) -> steamboots
and then it's depending on the role you play for your team, if I'm most time tanking i start with a cheap tank item, like helm of the black legion/ shaman's headdress (if the got many casters)
If You don't have to buy tank items just go for attackspeed, because of Gore
build first some cheaper items like Alchemist's Bones (which helps u farm alot ~ 300 g for some creeps) and later go for luxury items like Savage Mace/Frostburn/Breastplate etc.
Most time the game is allready over and a still have:
Power Supply, Steamboots, Alchemist's Bones, 2x Bracer, 1x TP
if you can farm good enough you will fast have the money for Savage Mace but if you have the right combo of heroes (pesti and panda laneup for example) game will be ended before you finished it ^^'
And Shrunken Head is first item if you got enemies like Moonqueen, Swiftblade, Keeper, Glacius, Plague Rider etc. I buy it as first luxury item after boots if you got enemies like mentioned.

Interesting, I've been playing him wrong this entire time, I guess. I usually go starting items -> marchers -> insanitarius -> finish steamboots -> portal or shruken, depending on how many disables the enemy team has.
After finishing Steamboots I'd usually just roam around with someone and gank people, which made the next core item come along pretty quickly, but I'll have to try putting off Insanitarius until later next time I get him.
Power supply definitely.
Now if you are dominating your lane, go for early insanitarious before boots even. If not, make steamboots and go directly for shrunken head. After you've got shrunken its either Shieldbreaker(cheap, but gives best dps) or Daemonic breastplate(expencive but tanky).
Of course, buy a power supply early. Most cost effective item in the game and it builds up from minor totems and a mana battery, items you should be buying anyways.
BKB should usually be first big item, sometimes I even end up going for it before upgrading boots. It's just extremely powerful. Portal Key MAY be grabbed before BKB depending on the lineup (low disable means you can put BKB off a bit, high disable means you put blink off, if you're the only initiation on your team you always go blink first).
I personally still use treads on pesti but some people use phase. You don't really need the movespeed boost on phase because you're still going a hell of a lot faster than anything short of warbeast in his ulti or corrupted disciple. The STR and attack speed from treads also suit him better than the raw damage of phase boots.
As far as Daemonic is concerned, the damage and -armor from Shieldbreaker is more effective. Unless you already have someone going shiledbreaker on the team I'd always get it before Daemonic.

3 Routes I've seen for pestilence.
1. Insanitarious. Boots --> Steamboots/Insanitarious --> Insanitarious/Steamboots --> Whispering Helm --> Shrunken Head. Whispering helm adds damage, armor, and survivability to cover up the HP loss from Insanitarious
2. Portal Key. Steamboots --> Portal Key --> Shrunken Head. Portal Key allows Pestilence to initiate a lot better. Shrunken Head helps him stay in the fight and not get instagibbed once enemies can survive .
3. Shrunken Head. Steamboots --> Rushed Shrunken Head. If the enemy has a lot of disables, this helps him not bite the dust every time he tries to stun. More of a counter than a visable build.
Notes: Pestilence isn't very tanky at all. However, he's a carry/ganker before he is an initiator in my opinion. Building Headdress is going too far, I think I'd settle with mystic vestments.
Daemonic breastplate would go after the shrunken head in build #1, or after one more item (Probably a shieldbreaker) in build #2.
First of all, pestilence is not a carry.
Secondly, learn to buy portal key on him and you will see another way of playing him, the right way.
After portal key, get shrunken head. But please bracers are super super important on him.
Why get a hatchet? Get some gauntlets that provide you with more hp and damage. Hatchet is such a waste of money on most heroes, cept BH.
I end up playing Pestilence in alot of games and I don't really see the appeal in Portal Key on him. With Flight turned on and Steamboots, you really can chase down or run away from anybody. In team fights, Shrunken Head and Flight together allow you to initiate with an Impale fairly easily. I'm not going to say Portal Key is a waste of money because I've seen other players use it to a large degree of success, but for me, it just feels like 2k gold I'd rather have put towards another item that will increase my damage or survivability.
Port key can be taken if your team lacks initiation, but otherwise there are other things to get.
Sometimes I forgo attack speed almost completely, and do something like
Hatchet>Steamboots>Insanitarius>Shrunken Head>Shield Breaker> Rifts
if you are not the primary carry buy a port key after steam boots and a bracer, its a brutal item on pesti, every time the port key comes up you'll have a stun for it, portkey in, stun, ult and then start beating on them and its gg. They will try to run but you fly nigga so good luck.
after port key i like to go shrunken then shieldbreaker/demonic/insanitarius depending on what the team needs
10/10 people who build insanitarius on pesti fail horribly bad. Seriously, you just farm up that thing and waste time not ganking? And good luck on ganking with the components of it. The only build I've been followed time after time is Stats items> steamboots> shrunken> heart. I don't see the point that ganking with steam staff/helm/gloves is better than with a mighty blade. You play pesti to have a strong ganker/semi carry and somewhat an initiator, so stop farming crappy items hat delay his ganking power. If you want a carry, there's a ton of better choice.
SoR makes him nearly impossible to kill after Shrunken/Key
You seem like one of those nerds who plays nothing but EM though, where item choices don't even matter.
Ill go with power supply as the emergency potion, abyssal (helps a lot in early to build it (hp regen with the trinket and mana regen with the scrab) then shrunken or rushing shield breaker, depends the situation, frm there its downhill, rifts and/or savage mace and gg
AND NO pest i isn't a tank, i mean come on with would a tank have a +10% dmg recieved skill?
U can wing it and semitank on early maybe getting to a lifetube to make it an axe or nullplaying and playing tank-like with the shrunken head but thats as tanky as it gets.
Why not demonic breastplace? $ thats why
