When to build it? I'm don't know how it works, it wasn't in dota.
In my opinion:
-In noob games, nobody see the + and you don't except if you have a 600mana ulti
-In mid game, it's a passive bonus, and nobody can use it on purpose
-Is it possible in good games to really use it as a real hp rege and to do very great with it?
-Is it worth on hero due to poor stats win?
-Is it worth in mid game due to cost?
It can add up in teamfights to quite a bit healed overall. Sometimes if my team doesn't really have many casters/high mana abilities I'll swap it for an Astrolabe.
The only hero that I consistently build it on is Accursed.
Awesome item on Ophelia and SoulReaper and other support heroes.
Everyone has spells, so everyone can use the bonus. the mana regen and the armor aura bonus are quite nice as well. And it gives relatively decent mana regen for a pretty cheap item.
I like it on DSham, kind of makes his heal feel less redundant late game.
really strong item.. arguably overpowered.
Static and percentage mana regen, with 16 INT make this item a early-midgame DREAM for int mana hog spammers like
Additionally, it grants 10 damage, small bits of healing that will give you an extra 100 hp now and then, and 4 armor which the above characters need badly.
The aura also stacks with ring of the teacher :O and +1 static mana regen to allies is a lot! It's like half a glacius aura!
When to get this item? Get it as your first item as soon as your basic survivability needs are met. It's that good
Alot of people don't see the +1mana regen and +4armor. I almost always build this item on KOTF since it eliminates all mana problems and the +4armor plus his 3rd skill is 16 armor.
It also gives really good stats along with being awesome for spell spammers. Spell spammers being heroes like Dshaman or Accursed. It's awesome to have it on your team ecspecially if there's heroes like Madman, Dark Lady, Voodoo Jester, Panda, Armadon or Wretched hag on your team.
If you build it as one of your first items the amount of healing it's done by the end is massive.
To the above, the teamplay benefits are armor and static mana regen, and a tiny bit of health. the greedy effects of heavy mana regen are the main reason to get it.
I don't think you understand. You keep mentioning the heal from it. Ignore the amount of healing you get from it and it's still a good item.
Armour aura and 1 mana/second for the whole team is GREAT. And you basically pay 260 gold for a manatube. It's cheap, it's useful, it keeps your team alive, it will win you fights even if you're too dim to understand how it does that. It's everything a good support needs or wants except a disable.
It's not strictly superior to Astrolabe, but it does give better stats and gives mana instead of eating mana, so you don't always want Astrolabe anyway.
I mean, what I'm not trying to say is, it's not "just a random teamplay item", it's one of the BEST teamplay items. This, Astrolabe, and Barrier Idol, and Barrier is situational.
If you don't care about support having their spells available all the time due to an item that combines a good early game item (Ring) with a pseudo-bracer (Major Totem), both of which are good on support anyway, and then you invest 1260 on top of those (one good teamfight+push and you should be able to buy it!) and you're set for items for the rest of the game usually.

When you need mana regen and team support in the same package. For Plague Rider and Nymphora Astrolabe IS superior because they have max mana all the time anyway. But for spammy people like Jereziah, Hellbringer, Witch Slayer, Demented Shaman, Soul Reaper, Accursed, Nome's is very efficient. (Hell whenever you can you should buy Astrolabe AND Nome's)

Nome's Wisdom gives an aura of 4 armour and 1 mana regen, while giving you good stats (+6 to each and another +10 int on top of that), +100% mana regen, and it has a very easy buildup (one piece that's 1000 is sort of big, the other pieces 540, 325+175, 450, 260 are all easy to pick up). The recipe upgrades just about everything about it - +2 all stats on the totem, +4 damage, +1 armour, and +0.35 mana regen on the ring, and then it gives an additional +100% mana regen (a free mana tube!) and the minor "regen" on the aura.
The fact that it heals during pushes is, well, nice, but it's not why you get it.

Light rain said what I was about to say, it is a great overall item to boost casters that have a decent mana pool but not much in the way of regen. I like this item on soul reaper quite a bit for the regen. It allows you to stay in a lane forever. =>
Then, why you get it? It's just a random teamplay item you get and you don't know why? I feel like the heal is so small except on witch or pyro... It's like you can heal a madman for 150 hp in a whole teamfight.... Who cares?
Or mb it's a support item for supports who use a real amount of mana... but again, who cares?
I always get it on Wretched Hag. Feels like a lucky item. :3
Anyway, its more of an upgrade to Ring of Teacher.
If my hero who crap armor below 2 points, I get armor ring then upgrade to Ring of Teacher. I find that I survive better with armor than HP.
Major totem is like getting a bracer. Extra intelligence is always good.
What you do with Ring of Teacher is like what you do with those totems and power supply.
