You went out for a holyday break and people were killing each other.
This is strange, I can run both all_alt and no_alt without an error on Notausgangs ModMan. There is an issue with The JavaModMan (known) but this will be fixed in 0.9.6.
You said you reinstalled EVERYTHING... are you on Garena.HoN?
Two things I can think of
a) You have .s2z custom hero models (remove them)
b) You use .honmods changing string files
Could you download e.g. XXL Big Hero Guide and try to apply this mod?
If this doesn't work your Net.Framework is corrupted

i am not sure if you can identify if there is a certain hero or aspect of this mod that might effect the mario sound mod, but after updating to the newest alt hero mod i got this error.
"could not find string starting with "<frameevent frame="1" cmd=playsoundlinear sounds/death.wav 400 1800 .85"/>" as sough by line 38 of ......mariosoundmod.honmod....."
was just wondering if there is a certain hero skin or something that i might be able to disable or something to try and get both working. i also posted there as well.

At last, I have found the solution. The game does not display preview of pesti(bag with textures mb, she was like red and black texture), so when I disabled it in the "alt-models", modes applied successfully! Do not know whot called this problem, but next time I will also check in-store preview all of the alt-avatar)

i just tried this out...
for starters i had everything at same place (folder) Hon/game/mods right?
what i did was just removing the heromoldels 2.22v and ionic.dll to another folder at desktop and tried again... it simply worked out
btw u need to generate it then u move the heromodel.honmode back to Hon/game/mods fyi.
try it out and tell me if works..

BTW, that bug that you said "widescreen", is just the option to zoom windows of Windows 7 located in Control Panel\Appearance and Personalization\Display
