Jester throws an acid cocktail at Hellbringer. The cocktail hits Hellbringer, then splits in 2. The two cocktail then fly simultaneously to different targets. Each new cocktail bounces a final time.
It seems like Hellbringer tried to cast Life Void at the moment the initial cocktail hit him. Maybe this has something to do with the glitch?

It splits if the target enters/exits fog on the same frame.
Known bug is known but since the forums crashed I have to reconfirm it
