Add item skills to end of skill bar

Would it be possible to have a custom skills bar so that once an item is added to your inventory that has use such as shrunken head for example, it would add that icon next to your last skill, as well it will also extend your hot keys to something like qwer-t or whatever you have it set to.

why would you do that? you can rebind your hotkeys for item usage of specific slots to T,Z,... and move your useable items to that slots.

Quite frankly, I think that is a much more organized approach.
It would organize all of your active abilities in one group, rather than separating your item abilities from your hero abilities.

Exactly, basically someone just needs to create like 3-4 invisible boxes next to your current skills and make it so you can move your items into those slots in whatever order you want.
