I've been unable to get definite answers on these, and i'm terrible at navigating forums, so if you'll indulge me.
Lifesteal, how does it actually work? is it %of damage done is returned to you as raw HP? and does it work for everyone, or just for melee (Like bash, which has smaller chance for ranged). Also, the cleave effect, does that work for ranged? Seems a little like all the cool attack modifiers are melee-only

Most attack modifiers work for both melee and ranged. Bash and Hatchet give diminished benefits for ranged and Cleave doesn't work on ranged at all. Otherwise, the effects are identical whether you're ranged.

Depends on which mechanics you're referring to.
Abyssal Skull gives a lifesteal aura which only works for melee heroes, but the other lifesteal items work for all heroes with no reductions. The lifesteal items regenerate HP by percentage of damage you deal from attacks - not spells or abilities.
Some pseudo lifesteal skills like Predator has, doesn't regenerate HP the same as the items - his regenerates the % of current HP your opponent has when you strike them. Lifesteal and his skill stack with eachother, so you can regenerate HP from both sources whenever you attack.
Cleave only works for melee heroes.

The following attack modifiers/items do not work for ranged:
Abyssal Skull
Cleave and Runed Axe
The following attack modifiers/items have a reduced effect for ranged:
Bash and Brutalizer
Logger's Hatchet

I'm pretty sure its not damage done, but damage received. I mean, it's your damage less the reduction effect from the enemy's armor.

Lifesteal is an exlusive modifier which mead you can only have one item with the lifesteal orb working at one time. There is only one item that breaks this rule; Abyssal Skull is an item that provides a lifesteal AURA. Auras effect everyone in it's radius and is completely seperate from attack modifers.
Lifesteal hp return depends on damage dealt to the target after everything.
Lifesteal works the same way for both ranged and melee except for one rulebreaker item again; Abyssal Skull ( oi this item breaks a lot of rules) Abyssal Skull's lifesteal aura only effects melée characters.
Cleave will only work for heroes that have been stated to be melée.
Cleave does unreducable damage to enemies behind a target that is hit with the normal attack. Damage dealt by cleave will not lifesteal in any way. All cleave effects are completely stackable.

yeah the vamp % is based on the value of damage you deal AFTER armor reduction is applied
i.e.
technically you can throw in other variables from shields and other random garbage, but that's the basis
