I guess you are familiar with this issue, so I don't need to explain all too much. However, I'll still do a thorough explain of what I'm experiencing.
Setup
OS: Windows Vista™ Ultimate (6.0, Build 6002) Service Pack 2 (6002.vistasp2_gdr.100608-0458
MB: Asus P5K-E
CPU: Intel Core2 Quad CPU Q6700 @ 2.66GHz (4 CPUs), ~2.7GHz
RAM: 8GB
GPU: NVIDIA GeForce GTS 250

Normal/Regular FPS: 60-70
Normal temperatures and CPU usages according to SpeedFan + PCProbe II.
Estimate Occurrences: Once per game at a minimum. Often/mostly 3-4 times per game.
Mods: None, never used any.
Installation: Re-installed a couple of times.
Explanation of the issue
It happens randomly during a game session, without any kind of sign or warning. My screen goes black, followed by a little popup message from the "tray-bar/systemicon's" basically saying that my graphics driver has been reset. It's black for about 2-3 seconds before returning to normal.
I'd say that 60-70% of the occurrences includes a nasty FPS drop. Which leaves me with a constant 16-17 FPS for a time period of about 1-5 minutes. It returns to normal by either waiting it out or restarting the game.
50% of the occurrences also includes a "sound failure" of sorts. Hard to explain, but the sound gives off a lot of "scraping" noise and sounds almost like something from a NES or older quality. Though it goes away after about 30-40 seconds. This affects everything, not just the sound from HoN. (e.g Skype, MSN etc.)
These occurrences can happen very quick, almost instantly, after eachother. Once I waited out the FPS drop from a blackscreen, and just a few seconds after the FPS got back to normal I got an additional blackscreen with a FPS drop included. (Though I'd say that the dual-blackscreen-combo is a pretty rare phenomenon)
It goes without saying that these occurrences have either ruined and/or affected the nature of the game quite drastically, many, many times. I'm just praying that it happens when I'm "safe" and not during combat.
I have been having this issue for as long as I can remember. I know the issues lies with Nvidia, but I thought I'd write a bug report anyways. Since I'm quite disappointed because, obviously, it has ruined some/a lot of my passion and appetite for the game.
Was to include DxDiag in the attachments, due to .rar not being allowed and .txt size limitations, it couldn't happen.
Edit: Trying to add a console.log which contains this "crash", however, I can't. "Your file of 1.26 MB bytes exceeds the forum's limit of 97.7 KB for this filetype"
You need to look over your attachment restrictions.. :/
