This seems to happen when I play HoN for a while, or alt+tab out of the game a couple of times. It does not always happens, but it does happen very often. It happens in game, as well as in Windows.
I can't take a screenshot of it because screenshots don't include the mouse...
But it basically looks like missingno as a square (http://www.gamepadmagazine.co.uk/wp-content/uploads/2010/11/missingno.jpg). Or sometimes as a pointer that has been hacked to pieces and quickly put back together as a Frankenstein pointer.
Please make it stop. =(
