In the item tooltip it says a damage shield is applied for five seconds if the heroes health is full, but the buff actually lasts ten seconds

Confirmed.
Just to let you know, initially it buffs you for 10 seconds. If you cast another spell while at full HP, it won't refresh it to 10 seconds if you're over 5 seconds duration remaining. If you have less than 5s on the buff, it'll refresh it to 5s
